Physician owned medical group acquires First California Physician Partners primary care practices in Modesto and Manteca
MODESTO, Calif. (December 19, 2024) – HealthyU Clinics, a leading provider of integrated primary and specialty care services, announces its expansion into the Central Valley with the acquisition of three First California Physician Partners (FCPP) primary care practices. Effective December 9, 2024, two FCPP- Modesto family medicine practices, located at 3125 Conant Avenue and 1541 Florida Avenue and one FCPP- Manteca internal medicine practice, located at 1262 East North Street will be rebranded under the HealthyU name while continuing to offer primary care services.
HealthyU Clinics has committed to retaining nine physicians and over 40 staff members to ensure patient care is the highest priority during the transition. The physician-owned, professionally managed medical group expects to maintain current insurance partnerships and keep all appointments as scheduled.

In Modesto, HealthyU Clinics will maintain the office at 3125 Conant Avenue, where Dr. Giang Lam and Dr. Nishant Patel will continue to provide high-quality healthcare for existing and new patients. They will relocate the clinic located at 1541 Florida Avenue, moving the team just two blocks to its new location at 1444 Florida Avenue, Suite 100 in Modesto. The trusted care team will include Dr. Raul Montalvo, Dr. Bradley Blankenship, Dr. Barbara Kennedy-Funtila, Dr. Humayun Abbas, and Dr. James Boozer.
In Manteca, HealthyU Clinics will keep the FCCP Manteca Medical Group office located at 1262 East North Street, retaining Dr. Terry Croasdale and other care providers.
